【摘要】 目的 比较不同种类降压药物单用及联用对中心动脉压的影响。 方法 选取未服药或正在接受药物降压治疗的139例原发性高血压患者,根据所服药物进行分组,分为未服药组、单用钙拮抗剂组、单用β受体阻滞剂组、单用血管紧张素受体阻滞剂组(ARB类)或血管紧张素转化酶抑制剂组(ACEI类)、联用钙拮抗剂和血管紧张素受体阻滞剂组(ARB类)或血管紧张素转化酶抑制剂组(ACEI类)、联用钙拮抗剂和β受体阻滞剂组,应用欧姆龙HEM9000AI进行无创性中心动脉压及外周肱动脉压的检测。 结果 单用β受体阻滞剂组中心动脉压及增益指数(AI)均要高于其他药物治疗组,结果有统计学差异(P<0. 05);使用β受体阻滞剂组中心动脉压及增益指数高于未使用β受体阻滞剂组,结果有统计学差异(P〈0. 05);单用ACEI类药物组与单用ARB类药物组比较,中心动脉压及增益指数无统计学差异(P>0. 05)。 结论 肱动脉血压并不能准确反映中心动脉血压;不同种类降压药物由于作用机制不同,对中心动脉压力及血液动力学有不同的影响;相对于其他药物治疗组,β受体阻滞剂美托洛尔、比索洛尔虽然降低外周肱动脉压的效果与其他各组相同,但降低中心动脉压的效果较差。 【关键词】抗高血压药物;中心动脉压;增益指数;脉搏波分析。
Objective—The purpose of this study was to compare the effect of different types of antihypertensive drug classes alone or in conjunction on central aortic pressure. Methods—139 patients with essential hypertension who were not taking drugs or receiving antihypertensive therapy were selected, according to what medicine he was taking, they were divided into non-medication group, calcium channel blockers group, β-blockers group, angiotensin receptor blocker (ARB) or angiotensin-converting enzyme inhibitor group (ACEI), combined with calcium antagonists and angiotensin receptor blocker group (ARB) or angiotensin-converting enzyme inhibitors (ACEI) group, combined with calcium antagonists and β-blocker group, then central aortic pressure and brachial artery pressure were detected by HEM9000AI(Ormon). Results—The central aortic pressure and augmentation index of β-blocker group were higher than the other groups, resulting in significant difference (P <0.05); The central aortic pressure and augmentation index which used β-blocker was higher than the group which didn't use β-blocker; The central aortic pressure and augmentation index of ACEI group and ARB group had no significant difference (P> 0.05). Conclusions—Brachial artery pressure can't accurately reflect central aortic blood pressure. Different types of antihypertensive drug classes may have different effects on central aortic pressure and central aortic hemodynamics; Metoprolol and bisoprolol reduce central aortic pressure less effective than the other drugs, although they can reduce brachial artery pressure the same extent like others.
